Preaching a Personalized Touch in Healthcare for a More Engaged Patient Experience

Pager Health, a connected health platform company serving more than 26 million members in the United States and Latin America, has officially announced the launch of its latest solution in ReallyWell.

According to certain reports, the stated solution combines clinical support, wellness services and contextual navigation, as well as leverages AI to conduct deep-dive analyses of all-encompassing data sets for each member and patient. This way it createsan unprecedented, one-size-fits-one personalized engagement experience for healthcare. More on the same would reveal how it provides clinically prioritized wellbeing journeys that guide each member, step-by-step, to better health, providing relevant content and tailored recommendations which dynamically change with the member’s health status,and therefore, deliver core value. 

Not just that, ReallyWell is also the first platform to seamlessly integrate clinical support, wellness services,and contextual navigation across the entire continuum of care. Empowered by one-click, omnichannel access to a nurse or health coach, the solution makes it possible for any member to find the right resources or care protocols whenever they have a question or medical concern. Such an immediate and seamless connection to holistic care, like you can guess, eliminates the navigation issues created by the otherwise siloed healthcare landscape. Furthermore, it supports proactive health management through early awareness and timely intervention whenever new medical issues arise.

To understand the significance of such a development, we must acknowledge how, on an average, users of wellbeing apps spend only two to three minutes per day, and more than half of these apps are uninstalled within 30 days of being downloaded. Against that, ReallyWell has shown to engage members in their healthcare, generating an 83 percent gap closure rate for digitally opted-in wellness exams and an 86 percent sustained engagement rate.

ReallyWell has already achieved a member satisfaction score of 88 percent, a powerful demonstration of the potential of this highly personalized health experience.”

Talk about the solution’s capabilities on a slightly deeper level, we begin from its Wellbeing Copilot which, for each member, consolidates and synchronizes wellbeing and healthcare interactions, claims, advanced biometrics, clinical risk factors, SDOH attributes, behavioral needs, and other clinical data into a consolidated data profile. Banking upon the stated data, this GenAI powered wellbeing copilot is able to provide highly contextualized content, health and wellbeing recommendations and a “next-best-actions” care journey for each patient/member, creating a relevant, real-time personalized engagement experience.

Next up, we have an element rooted in Health Risk Assessment (HRA), a NCQA-accredited Health Risk Assessment and flexible survey tool bringing to the fore an enhanced question set which reduces completion time, generating a 59% completion rate, to help quickly guide each member to the right level of care, at the right time, and at the right place. On top of that, Pager’s HRA can also be configured and deployed to meet a multitude of other data-gathering needs like improving population health by identifying the specific health issues that are impacting high-risk groups.

Rounding up highlights would be a dynamic wellness score, which includes an overall wellness score for each member, as well as a score for each component part. Markedly displayed in a visually intuitive way, the dynamic wellness score responds in real time to actions taken by members as they make their way through next-right-thing-to-do recommendations. By doing so, it encourages members, who always know exactly where they are on their wellbeing journey.
